You can go to any primary care office and negotiate a cash rate. Many give a 30-50% discount off their list price if you call and ask before you go.
But the real answer you’re probably looking for is to go to a community health center (CHC). A community health center is just like any other doctor’s office, but subsidized by the federal government to provide access to primary care services.
They have a “sliding fee scale”, where you pay according to your yearly income. Bring three pay stubs, or your last year’s income tax or W-2. If you do not have those documents, you must bring something to show how you are supporting yourself. Here’s a sample of how much you may have to pay. CHCs also have dental services as well on the sliding fee scale. They’re the hidden gem of the US health care system (for all it’s brokenness). The staff and physicians who work there are the greatest because they are fulfilling a calling to serve.
